Buffalo Medical Group is pleased to offer our patients on line access to portions of their medical record. If you would like to participate as a user of MyChart you may sign up for the service by completing the registration form at the end of this section.

You are being provided access to the MyChart information system through Buffalo Medical Group. To access MyChart, the user must agree to be bound by the terms of this MyChart Licensing Agreement for the material and data contained in MyChart. If you do not agree to the terms of this Agreement, please do not use MyChart.
I understand that Buffalo Medical Groupís MyChart should NEVER be used for urgent health care matters. The anticipated turn around time for a response to a secure message may be 2 or more business days. Therefore if I believe I am experiencing an urgent or life threatening health condition I should phone my physician, go directly to an Emergency Department of a local hospital or call 911.

Please note that MyChart may be viewed by other health professionals and others who work with your physician.

Medical Information

Information on MyChart is provided "as is" without warranty of any kind and is meant for use only to support the patient's relationship with their current or future medical professional. User reliance on the material in MyChart is at your own risk. Please note the medical or health care information provided on MyChart is not a replacement for proper medical attention. The electronic record is the property of Buffalo Medical Group and the information displayed in MyChart may not be the complete medical record. If you are in need of copies of your complete medical record you must contact Buffalo Medical Group Medical Records department and complete a HIPAA compliant release of information request form. Please contact the appropriate physicianís office directly in regard to discrepancies with the medical information listed in your record.

Buffalo Medical Group places considerable emphasis on the privacy of patient information and their compliance with State and Federal Privacy Laws. You understand that you are solely responsible for any sharing of its content that you intentionally or unintentionally communicate to others. Please do not share your password with anyone. MyChart is configured to be secure from unauthorized access, but Buffalo Medical Group cannot be held responsible for:
a) Absolute security of all electronic communication transmissions between patient and the site.
b) Unauthorized disclosure resulting from a user not logging out of an active session;
c) Unauthorized disclosure resulting from a lost or stolen user ID and password;
d) Unauthorized disclosure resulting from information printed by the user from MyChart;
f) Unauthorized disclosure resulting from personal computer settings or installed software products that may compromise information security; or
e) Similar events outside the substantial control of Buffalo Medical Group.


Security

∑ Encryption: We use a 256-bit encryption to encrypt communication from your web browser to our secure network web server.

∑ Buffalo Medical Group uses a dedicated web server. We separate the web server and the electronic medical record. In the unlikely event the web server is compromised, no health information would be exposed since it is not saved to the web server.

∑ Electronic patient information is saved behind a firewall.

∑ No caching of data in the browser is allowed; we disable caching and force the web pages to expire immediately.

∑ After a period of inactivity the login session will time out.

∑ We constantly monitor the web server for evidence of any break-in attempts.

Appropriate Use

Access to the MyChart system is granted to users for the purpose of furthering their medical care. Use of the system unrelated to this purpose may result in the suspension or termination of access privileges. Improper activities include, but are not limited to, as the use of inappropriate or abusive language, reserving appointment times that are not needed or are frequently cancelled and other activities that are deemed to cause a disruption to clinic operations in the sole discretion of Buffalo Medical Group.

Other Terms

You can disenroll from your use of Buffalo Medical Groupís MyChart at anytime by completing the appropriate online request form.

Buffalo Medical Group shall not be liable for any damages suffered by the user or other third parties related to, directly or indirectly, the material and information contained in MyChart. In no circumstances will Buffalo Medical Group be liable to the user or any other person for direct, indirect, special, incidental or consequential damages of any type, even if informed of the possibility of such damages. Buffalo Medical Group cannot guarantee access to the MyChart system and are not liable for the user's inability to access or view the information contained on the site. By downloading or viewing the information on MyChart, the user agrees to hold Buffalo Medical Group harmless and indemnify them against any damages, claims, or other claims asserted against Buffalo Medical Group arising out of, or in any way connected with, the user's utilization of MyChart or the information contained therein.

This Agreement shall be governed and construed in accordance with the laws of the State of New York. MyChart information and the logos, icons and other marks shown or referenced within are reserved by Buffalo Medical Group and are protected by copyright and trademark law. The user may not reproduce such information without the prior written consent of Buffalo Medical Group. This Agreement contains the entire agreement between the parties with respect to the subject matter contained herein and it shall not be modified or amended except by signed written instrument. The terms of this Agreement may be changed at any time in the discretion of the Buffalo Medical Group and updated terms will be posted on the MyChart website for user access.
